Required Forms For Service
We kindly request that all necessary forms are filled out before service. These forms allow us to go into your treatment with a sustainable action plan, and save a great deal of time. We will provide physical copies if requested. We are not liable if a service cannot be performed due to a contraindication that was not mentioned. If you do not feel comfortable filling out a photo release, simply skip that form, however we do appreciate being able to share client progress and performance.

First time facial clients- please complete first time client form, and photo release.
First time peel clients- please complete first time client form, chemical peel consent, post-peel care consent, and photo release.
Returning facial/peel clients- please fill out returning client form.
First time eyelash extension clients- please fill out eyelash extension consent form and photo release.
First time wax clients- please fill out waxing consent form and photo release.
NOTE: If you are under 18 years of age at time of service, please have a parent fill out the minor release forms. Chemical peels will not be performed for those under 18.
